Application Form:

This Application Form needs to be downloaded, filled out, and mailed or faxed to the college. You will need to have Microsoft Excel or an equivalent spreadsheet application.

Academic Requirements

High school Diploma (equivalency) including English30 (33), Math 20(applied), Biology 30

If you are a student who does not have a minimum of 3 years of full time study in English, submit one of the followings as a proof of English language proficiency instead of English 30(33)

1. Minimum of 75% in CDTC ESL Assessment Test
2. TOEFL (Test of English as a Foreign Language):
1. iBT 80 with no individual test component less than 20
2. PBT 550
3. CBT 213
3. IELTS (International English Language Testing System): Overall score of 6.5 with no band less than 5.0
4. CAEL (Canadian Academic English Language Assessment): score of 70
5. MELAB (Michigan English Language Assessment Battery): score of 85
6. CDTC ESL 100/200: Successful completion

Manual Dexterity Test

This is going to be a good tool that students can check their manual dexterity.

The technology requires a high degree of technical ability and manual skills along with scientific knowledge and a degree of artistry

Candidates for careers in dental technology possess good eye-hand coordination and color perception, dexterity in using small instruments, the patience to attend to minute detail. The dental technology program offers a challenging career for those individuals interested in applying artistic and creative talents in a health care laboratory situation.

Immunization Records

All students accepted into dental technology program must have the documented proofs of Measles (Rubeloa), Mumps, Rubella (German measles), and Hepatitis B.

A copy of your records may be obtained from your local public health clinic or physician's office.

Application Procedure

1. Check your entrance requirements
   • I have high school diploma and
   • I have the credit of English 30(33), Math 20(applied), and Biology 30. Or
     I don't have the credit of English 30(33) but I have the equivalency as follows:
     • I will write the CDTC ESL Assessment Exam. Or
     • I have the other assessment test mark. Or
     • I will take CDTC ESL course.
   • I have Manual Dexterity Test result or I will have it
   • I have the documents of vaccination of Measles, Mumps, Rubella, and Hepatitis B or will have them
2. Submit the Application Form with application fee.
   You will need to have Microsoft Excel or an equivalent spreadsheet application.
